· Referrals to social service programs

· Public Health Nurse services: home visits, health assessments

· Nutritional education services with Registered Dietitian

· Diabetes self-management program

· Exercise groups and events

· Walking Program

· Immunization Services

· Transportation for Health and Wellness related appointments

(No one will be denied access to services due to inability to pay, and there is a discounted/sliding fee schedule available based on family size and income.)
